Participant Waiver & Safety Agreement

By registering my child for Northern District Hockey Club’s Come Try Hockey, I give permission for them to participate in the hockey activities provided.

I understand that hockey is a physical sport and, like all sporting activities, there is a risk of injury. Participants must follow the instructions of coaches and volunteers and behave safely and respectfully in line with applicable Hockey Australia and Hockey NSW rules and policies.

🏑 MOUTHGUARDS ARE MANDATORY

All children must wear a properly fitted mouthguard to participate in on-field hockey activities. Children without a mouthguard will not be permitted to participate.

Participants should also wear shin guards and enclosed sporting shoes. Hockey sticks can be provided if required.

In the event of an injury or emergency, I give permission for appropriate first aid or medical assistance to be provided and for me to be contacted as soon as possible.

By completing this registration, I confirm that I am authorised to give consent for this child to participate and agree to the conditions above.
