NDHC School Holiday Clinic – Participation Waiver

By registering for the NDHC School Holiday Clinic, I confirm that:

• I understand participation in hockey involves inherent risks, including physical injury or accidents.
• I release and waive any claims against Northern District Hockey Club, Hockey NSW, coaches, volunteers, sponsors, and venue owners for injury, loss, or damage, except in cases of gross negligence.
• I agree to indemnify the organisers for any damage or injury caused by my or my child’s actions.
• I confirm the participant is medically fit and able to safely take part in the clinic.
• I consent to photographs or video taken during the clinic being used for club or promotional purposes.
• I agree to follow all safety rules and instructions from coaches and officials.
• I understand the clinic may be modified, delayed, cancelled, or rescheduled due to safety, weather, venue conditions, or other unforeseen circumstances, with refunds or rescheduling at the organiser’s discretion.
• I acknowledge it is my responsibility to check official NDHC communication channels for event updates.

For participants under 18 years, I confirm I am the parent or legal guardian and accept these terms on their behalf.
